PETER J. COOK petition for adoption } In Common Pleas Court
of AMMA ELDORA HEYWARD } February Term 1873

PETER J. COOK respectfully petitions the court as follows. He respectfully asks to be permitted to adopt as his child one AMMA ELDORA HEYWARD who was two years old on the 28th day of September 1872. Your petitioner says that said AMMA ELDORA HEYWARD has no property either personal or real that she has a father living in Delphi [in Delphi crossed out] in Carroll County in the state of Indiana that she has no mother living. He respectfully asks that the said AMMA E. HEYWARD may be adopted by him and her name changed to AMMA ELDORA COOK.
